Please Use the Following Checklist and

Come Fully Prepared for your Appointment

 


  • Blank Check- a must for e-file and/or direct deposit
  • Copy of Tax Returns from the previous 2 years- If you are new to Stone Tax & Advisory Group
  • W2s- if you are still working
  • Self Employed- All records and receipts already totaled
  • All 1099s
    • CDs, Brokerage and/or Investment Income or Expenses 1099s
    • Pension 1099s
    • 401K/IRA distributions 1099s
    • 401K/IRA contribution 1099s
    • Social Security 1099
    • State Refund 1099-G
    • Received Unemployment Income 1099-G
    • Securities Brokerage 1099 for dividends, capital gains, redemptions, and Advisor Fees paid


  • 1098- if you paid mortgage interest
  • Charitable Receipts- if you made charitable contributions
  • Medical Expense Records- if you plan to itemize medical costs
  • Settlement Sheets- if you sold/purchased real estate
  • Schedule K-1- if you have K-1 activity
  • Paid educational expenses- supporting documents for post secondary education
  • Estimates Tax Payment Documentation- if you paid estimated taxes and dates payments were made
  • Calculated Total Alimony- if you paid or received alimony
  • Legal Names & SS# of Children and/or Dependants
  • Name, Address, and SS# or EIN# of Care Provider- if you have childcare or dependant expenses
  • Other Necessary Records may be needed if you:
    • Had a loss due to theft or casualty
    • Purchased a qualified Long Term Care Policy
    • Converted Traditional IRA funds to Roth IRA funds
    • If you were a resident of, or received income from more than one state during the year
    • Had non-reimbursed job expenses, if yes calculate totals
    • Bought, sold, or traded special assets
    • Had moving expenses
    • Made large purchases
    • Made home improvements (particularly energy savings)
    • Own vacation home that was rented to someone else at anytime
    • Have business income or are self employed
    • If self employed, and you paid health or long-term care premiums
    • Made contributions or received distribution from Health Savings Account
    • Your marital status has changed in the last year
